Quality of Life in children and young people with Congenital Adrenal Hyperplasia - UK nationwide multicentre assessment
CONCLUSIONS: In CYP with CAH, reduced height, increased weight, and hormonal biomarkers consistent with overtreatment were associated with worse QoL, and addressing these problems should be prioritised in clinical management.PMID:37439248 | DOI:10.1210/clinem/dgad405 (Source: The Journal of Clinical Endocrinology and Metabolism)

Pulsatile Subcutaneous Hydrocortisone Replacement in Primary Adrenal Insufficiency
In conclusions, mimicking endogenous cortisol rhythmicity by ultradian subcutaneous infusion of cortisol is feasible. It was superior to both continuous pump and oral therapy in maintaining normal ACTH levels throughout the 24-h cycle. Our results demonstrate a low free cortisol bioavailability on thrice daily oral replacement therapy compared to both types of subcutaneous infusion. Dexamethasone affects human fetal adrenal steroidogenesis and subsequent ACTH response in an ex vivo culture model
This study aimed to examine direct effects of DEX on human fetal adrenal (HFA) steroidogenic activity including possible effects on the subsequent response to ACTH-stimulation.MethodsHuman fetal adrenal (HFA) tissue from 30 fetuses (1st trimester) were cultured ex vivo with A) DEX (10 µm) for 14 days, or B) DEX (10 µm) for 10 days followed by ACTH (1 nM) for 4 days. DEX-mediated effects on HFA morphology, viability, and apoptosis (immunohistochemistry), gene expression (quantitative PCR), and steroid hormone secretion (LC-MS/MS) were investigated.ResultsDEX-treatment caused decreased androstenedione (p<0.05) and incre...

Prevalance of congenital adrenal hyperplasia (cah) carriers in pcos patients seeking fertility treatment
Although the underlying pathophysiologies differ, patients with polycystic ovarian syndrome (PCOS) and congenital adrenal hyperplasia (CAH) often present with phenotypic similarities due to androgen excess. While typically exhibiting milder symptoms, CAH carriers may also have an increased risk of developing hyperandrogenism. Though the genetic basis of PCOS is still unknown, it is possible that there are multiple etiologies to this complex and multifarious syndrome and even intersection with other endocrinopathies like CAH. Monozygotic twins with identical premature timing of acne onset: A Case report
This report describes a pair of monozygotic twin brothers with identical premature onset of acne, who were found to have an identical homozygous mutation in the promoter region of the CYP21A2 gene. While it is widely known that NCCAH is associated with genetic changes, the drive behind onset of adrenarche are widely unknown. As such, this report provokes thoughts on whether adrenarche could be influenced by adrenal genetic polymorphisms.PMID:37387304 | DOI:10.1111/ajd.14117 (Source: The Australasian Journal of Dermatology)
