Stimulant/Naltrexone for Weight
I've had some patients that benefitted from bupropion/naltrexone for weight loss. I got to wondering about my ADHD patients that are on methylphenidate or amphetamine (but not bupropion) if adding naltrexone to their regimen would be helpful for weight loss. Does anyone have experience with, or thoughts about, this? (Source: Student Doctor Network)

Thoughts on ADHD stimulant Rx with UDS positive/Cannabis Use?
I've spoken to a lot of psychiatrists, and read a lot of papers online past few days. Seems to be a lot of back and forth, whether or not reasonable to prescribe medications like Adderall or Ritalin for patients that use cannabis, particularly recreationally. Some state no way, others state treatment of ADHD obviously helps with substance use. Thoughts, or any good links for further reading? (Source: Student Doctor Network)

Adderrall nationwide shortage - has this affected you?
I've been seeing a lot of buzz online lately of Adderrall shortages, and have talked to some people who are really concerned that they may have to come off stimulants altogether. One person claimed their doctor didn't want to change their prescription at all. Has this affected your practice at all (have a lot of people come to you asking for changes)? Are you concerned about having to switch someone to Ritalin / Concerta / etc? Would that be difficult to manage in dosing / sides? Just... Read more (Source: Student Doctor Network)

Lamictal augmentation for unipolar depression
Covering for an NP today and her overall diagnoses and management is driving me f*cking insane, doing things like diagnosing BPD as bipolar, and starting Lamictal for irritability. Have seen multiple patients of hers with MDD only today, resulting in her starting low dose Prozac, Seroquel 25 mg QHS for sleep, and Lamictal AT THE SAME TIME. Here are 4 patients of hers I saw today, all MDD: Patient 1: Trintellex 10 mg once daily, Ritalin 30 mg once daily, Lamictal 150 mg Patient 2: Prozac 10... Read more (Source: Student Doctor Network)

What Are Your Thoughts on Prescribing Stimulants for newly diagnosed ADHD adults?
I'm starting a new outpatient job this week. Haven't done outpatient in a bit, was wondering what your thoughts are with : 1. Diagnosing ADHD in adults >21? 2. And prescribing stimulants (such as Adderall IR or Ritalin)? And if you do prescribe stimulants, what should I do to be comprehensive? 1. Neuropsych testing? 2. What screening tests should I use? CAT-A? TOVA testing? 3. Should I get UDS on initial visit? And if so, how often do you do UDS on patients Rx stimulants? Any... Read more (Source: Student Doctor Network)

medications for traumatic brain injury
I am taking care of a patient with a brain injury as an outpatient who has been taking ritalin and a SSRI prescribed by a previous physiatrist. I have used stimulants before for treatment of patients with arousal or concentration issues following brain injuries, but what about the use of SSRIs? Do they help with neuronal remodeling and neuroprotection following central nervous system injuries? I guess it would probably depend on their symptoms and response to those 2 medications, but how... medications for traumatic brain injury (Source: Student Doctor Network)

How do you deal with a$$hole patients. I encountered a witch looking entitled medicaid patient today who screamed on the phone for 10 mins for not having her methylphenidate ready. Wtf?! And I was like, b!*ch i am the supervisor. Lol Has anyone refused to fill a prescription or forced a transfer of people... Firing patient (Source: Student Doctor Network)

Is a career of exploring potential nootropic benefits of substances that are currently regarded as illicit possible in academic neuropsychiatry??
Examples (might be completely implausible but just to give an idea of what i mean): -Pure cocaine microdosing instead of amphetamine/methylphenidate for ADHD. -LSD/Psilocybin or MDMA microdosing instead of SSRI for depression Does the government allow you to conduct studies like that? Why can't pharmacists change brand name to generic?
Today I sent over an rx for Concerta to the pharmacy. This is in Texas btw. 15 minutes later I get a phone call from the pharmacist at CVS. Pharmacist: Their insurance won't cover brand name, can you switch to generic? Me: Why are you calling me? Can't you switch it over on your own? Pharmacist: Well not really... Me: Fine then, absolutely, go ahead and change it to generic methylphenidate Pharmacist: I need your approval to switch it Me: Why? Pharmacist: I cant change it... Methylphenidate for the poor, vs non-poor
are there more to this story than what's in the mail? Medicaid ppl are getting one version of methylphenidate, and we have to save that version for them. whereas the general public gets the other crappy version of methylphenidate that poor ppl don't even want. if they're paying zero, why are they so demanding? and is it legal to save the crappy version for the general public, and the better brand for Medicaid folks "only?" what's the real story behind this discriminatory behavior? Use of multiple benzos with stimulants
Hi all, I'm wondering if you could offer any insight about a patient who came into my pharmacy today with scripts for Ritalin 30 mg qid xanax 0.5 bid and klonopin 2mg bid. I really don't pretend to be a psych expert so i called the office and got a response of "just fill it". I didn't end up filling it but I guess my questions are where do you guys typically draw the line on titrating stimulant doses, what's the rationale behind prescribing stimulants with benzodiazepines, and what's the...
