Dyspnea And Skin Discoloration In A Transplant Patient — What’s The Cause?
What medication is associated with this patient’s presentation? A 41-year-old male patient presents to the emergency department with dyspnea, headache, vomiting, and weakness. He had a kidney transplant nine years ago, and has been on a stable anti-rejection regimen. He has allergies to ciprofloxacin and trimethoprim-sulfamethoxazole. Physical examination reveals gray-blue discoloration of the nose, fingers, and toes. A chest X-ray and ECG are both unremarkable. Arterial blood gas... Lack of education about fluoroquinolones?
I'm curious as to why this is taught so poorly, at least from my perspective. I didn't get much info in molecular pharmacology nor antimicrobial classes. even after asking the professors they shrugged it off. it seems like these antibiotics keep hitting the news, permanent nerve damage, disability from tendon rupture (not just achilles). I've trolled NCBI for specific studies and it's surprising how much information is out there on ciprofloxacin/levofloxacin etc that's not taught in any... Lack of education about fluoroquinolones? When I inquired about it, I was told "don't try it" -- almost in the sense of questioning his "medical judgement". To my knowledge, they were not a physician --- so -- how do the rest of you handle family members doctoring... Family members treating/loaning medications to patients? How do you know when you need IV antibiotics?
I have posted here before about my two sons getting diagnosed in their teens a few years ago. Because we did not go through the normal learning curve with CF, there is some basic knowledge that we do not have. This is one of these instances. My older son, age 20 is having an exacerbation. He has never been hospitalized (for anything, let alone CF). He has been on cipro over 3 days with no positive effect yet. He is using his vest 3 hours a day, and is coughing too much to sleep at night. When we called the doctor, he said all we can do now is take him to the ER if we want to. My question is -- what is bad enough ...
