Acute and chronic neuropathies
A detailed history, examination, nerve conduction tests and appropriate laboratory investigations will be diagnostic in most peripheral neuropathies. It is important to identify treatable causes. Guillain –Barré syndrome, vasculitic neuropathy, chronic inflammatory demyelinating polyradiculoneuropathy and multifocal motor neuropathy can have good outcomes with immunomodulation. In addition, hereditary transthyretin amyloidosis neuropathy is now a treatable disorder. (Source: Medicine)

Artefactually low creatinine by Beckman Coulter enzymatic method due to immunoglobulin M paraprotein interference
Ann Clin Biochem. 2023 Jul 23:45632231190507. doi: 10.1177/00045632231190507. Online ahead of print.ABSTRACTAn 81-year-old man was admitted to hospital with symptomatic coronavirus disease (COVID-19) infection. He had a background of progressive chronic inflammatory demyelinating polyneuropathy associated with Waldenstrom's macroglobulinaemia. His plasma creatinine on four separate samples was inconceivably low (all ≤13 μmol/L), as measured by a Beckman Coulter enzymatic assay) after being 72 μmol/L 3 months earlier. On further investigation, his serum immunoglobulin M (IgM) was 15.4 g/L and his plasma creatinine measu...

Repeated Acute Exacerbations of Chronic Inflammatory Demyelinating Polyradiculoneuropathy Accompanied by Pain and Swelling in Distal Extremities
Intern Med. 2023 Jul 19. doi: 10.2169/internalmedicine.2021-23. Online ahead of print.ABSTRACTAn 81-year-old man experienced acute progression of weakness in the extremities accompanied by a fever, tenderness, and swelling in distal parts of the extremities. He had flaccid tetraparesis with fasciculations and general hyporeflexia. Nerve conduction studies indicated demyelinating sensorimotor neuropathy. A cerebrospinal fluid examination revealed elevated proteins without pleocytosis. Immunological treatments were effective, but his symptoms exhibited repeated relapse and remission phases. He was diagnosed with chronic infl...
