Culture of medicine and time off
I’m wondering if this is just something personally engrained in me or if others can relate. I’m in my last year of psychiatry residency, recently had abdominal pain, vomiting and went to the ED. Had elevated LFTs, bili, lipase, WBC. Ended up with a 3 day hospital stay, ERCP with 2 stents placed and a cholecystectomy. I’ve taken no FMLA or extended time off my entire residency and asked for 2 weeks to recoup which ended up being granted. I’m 3 days out and feeling so incredibly guilty. I... What operations requires strict NPO?
I see surgeons order in NPO strict or NPO with meds and ice before surgeries, but there doesn’t seem to any obvious reason why they pick between the two. Some surgeons order NPO strict for laparoscopic cholecystectomy and others will be okay with ice and meds for the same procedure. I’m sure there are surgeries and risk factors that require strict NPO (bariatric, gastroparesis, etc), but is there any resources I can review to determine how this is figured out? Thanks in advance! Mass under sternum?
This patient is presenting with a 4 year h/o insidious in onset retrosternal noncardiac chest pain. Can anyone tell me what this is directly under this patient's sternum. Don't mind the air, he had just had a cholecystectomy. The air does however however help to demarcate the mass under the sternum which appears to be just outside the peritoneum. Looks the same on MRI and does not light up with contrast View attachment 223172 (Source: Student Doctor Network)

3 y/o post lap chole
Just checking how many have dealt with their kiddos having a cholecystectomy?? My 3 y/o son had his gall bladder removed at the end if October after having many issues eating. Biopsy showed chronic inflammation. Surgeon said this was common with CF kiddos. since surgery he is having some biliary spasms and severe back pain intermittently. His appetite is better overall, but wondering how long this might go on. He is on Levsin to help with the pain. Just checking to see how everyone's else's kids faired afterwards.
