CHAD-STOP for cardiac amyloidosis
CHAD-STOP is the mnemonic for the initial steps in the management of cardiac amyloidosis: C: Conduction and rhythm disorder prevention H: High heart rate maintenance A: Anticoagulation D: Diuretics STOP: STOP beta receptor and calcium channel blockers, digoxin and renin-angiotensin-aldosterone inhibitors Preload reserve is limited in cardiac amyloidosis due to severe diastolic dysfunction. Hence the only way to increase the cardiac output is by the heart rate reserve and hence it should not be cut down by beta blockade or calcium channel blockade. Renin-angiotensin-aldosterone inhibitors carry the risk of severe hypotens...

Mandated Queries of the Florida Prescription Drug Monitoring Program: A Three-Month Experience from a Cancer Center-Based Outpatient Palliative Medicine Clinic
This article represents the findings from the queries over the first three months ’ queries and brings further clarity to our initial findings. Methods This quality improvement (QI) project was reviewed and approved by the Orlando Health/UFHealth Cancer Center Joint Oncology Committee for 2018-19. We began recording results of all E-FORSCE queries occurring after the law ’s implementation of July 1, 2018 through September 30, 2018. We informed each patient that the PDMP query had become mandatory in Florida, and we discussed the results of each query with each patient. Each query examined the last 12 months of the patie...

Part 2 - We Were Wrong 20 Years Ago, Our Current Response to the Opioid Crisis is Wrong, But We Should Still Be Helping Most of our Long-Term Patients Reduce Their Opioid Doses
by Drew Rosielle (@drosielle)This is the second in a series of several posts about many aspects of my current thinking about opioids. The first post is here:Part 1 – Introduction, General Disclaimers, Hand-Wringing, and a Hand-Crafted Graph.Over-prescribing fueled the current drug overdose epidemic, and many of us who thought we were stamping out needless suffering contributed to the epidemic.A lot of what I read and believed about opioids early on in my career was wrong.I ’m old enough to remember those heady days in which there was a pretty large and ‘successful’ movement in American medicine to greatly liberalize...

Tartine ’s Basic Country Bread
I think I finally got this sourdough thing down. Check out this boule made using Tartine’s Country Bread recipe, the holy grail of sourdough. It’s the first sourdough recipe I ever tried, and now the best I’ve ever made. For those of you as new to this whole sourdough thing as I was just 6 months ago, Tartine is the bakery run by Chad Robertson in San Francisco, turning out small batches (only 240 loaves a day) of what many say is the best bread you’ll ever taste. Following in the footsteps of bakers like Nancy Silverton at La Brea Bakery in Los Angeles, Chad helped to put artisan sourdough ...

No, the HPV vaccine isn ’t optional
A lot of parents ask me, about the HPV vaccine,“Isn’t that the optional one?” Well, let me walk you through why my answer to that question is a hard“no.” Human papillomavirus (HPV) is a virus that causes warts and several types of cancer. There are over 200 different  strains of HPV, some of which […]Find jobs at  Careers by KevinMD.com.  Search thousands of physician, PA, NP, and CRNA jobs now.  Learn more. America's National Defense Strategy and the Paradox of Technology
The need for the military to make greater investments in technology is difficult if not impossible to separate from the risks posed by an increasing military reliance on this technology. Striking a balance between the two likely will not be easy. (Source: The RAND Blog)

Travel Ban Separates Thousands of U.S. Citizens from Their Spouses & Minor Children
President Trump announced the first version of his “travel ban” 2 years ago this weekend. The policy has already separated thousands of U.S. citizens from their spouses and minor children. In its current form, the travel ban blocks visas to nationals of five majority Muslim countries. By the end of this fiscal year, the government is on pace to separate an estimated 15,000 spouses and adopted minor children of U.S. citizens. While the State Department has not publicly revealed the exact figures, prior trends in visa issuances indicate that as of January 1, 2019, the travel ban had already prevented 9,284 spouses and ado...

Trump Cut Muslim Refugees 91%, Immigrants 30%, Visitors by 18%
ConclusionThe bottom line is that the Trump administration is leading a major overhaul in the types of travelers, immigrants, and visitors who are coming to the United States. His administration reduced Muslim refugees by 91 percent and has overseen a 30 percent cut to immigrant visas for majority Muslim countries and an 18 percent cut to temporary visas. These policies lacka valid national security justification, but they are nonetheless having a significant effect. President Trump is certainly following through on his promise to limit Muslim immigration, even if a “total and complete shutdown” has not happened.Table ...

Catching Up Before I Forget...
I have absolutely no idea what I have written to date.  Writing is so incredibly therapeutic to me, I am going to get back into the habit of writing regularly again. My divorce was final on August 24.  I thought I would be great, happy even.  I was SO wrong.  I didn ' t even know I was falling apart the week before the divorce was final, but I knew I felt emotionally overwhelmed.  I kept listening to the song " Over My Head (Cable Car) by the Fray, identifying with the lyrics, yet...it never clicked in my head that I was about to...I guess have a kind of breakdown I suppose?  No, not the breakd...

Mandated Queries of the Florida Prescription Drug Monitoring Program: Early Experiences from a Cancer Center-based Outpatient Palliative Medicine Clinic
This article describes our e xperiences in the first month of experience with the new law, although we plan to examine queries for a total of three months before closing this QI project. For the purpose of this QI project, we have documented patients ’ demographics, including each patient’s age, gender and limited identifying information, such as patient names and identification numbers; this data will be de-identified for any statistical analysis planned in the future. We also recorded patients’ main diagnosis and pain symptoms, the numbe r of prescribers listed by the PDMP as well as the dose of the patient’s opio...

On vaccines: 1 pediatrician vs. 13 celebrity opinions
I have to admit, I’m not the biggest follower of popular culture. I couldn’t pick a Kardashian out of a crowd if my life depended on it. I don’t know who Brad Pitt is currently married to, if he’s married. And I have no idea how many kids Natalie Portman has, if she has any at all. (In full disclosure, I was blanking on names and pulled up People.com to find some examples.) While I appreciate the music, films, and TV shows that celebrities produce, I’ve never devoted much mental energy to following their personal lives. And I certainly don’t turn to them for advice on, well, anything. But a lot of people do. Pa...

Travel Ban Cuts Immigration 93%, Travel 86% From Targeted Countries
The Supreme Courtupheld the legality and constitutionality of President Trump ’s travel ban this week, but it had alreadyallowed the ban to go fully into effect on December 4, pending its final decision. We nowhave five full months of data from the State Department to see how the ban has affected immigration and travel from the countries that the ban targeted. Overall, we have seen a dramatic decline in visa approvals, affecting most notably Iranians and Yemenis.Figure 1 shows the difference in the average monthly visa issuances for temporary visitors (i.e. nonimmigrants) and immigrants (i.e. permanent residents) for the...
