Prediction of central LN metastasis in PTMC
Central compartment lymph node metastases (LNM) is found in 40 – 60% of patients with Papillary thyroid microcarcinoma (PTMC). The standard care for LNM is total thyroidectomy with ‘therapeutic’ cervical lymph node dissection. However whether routine central lymph node dissection should be performed in the PTMC patients without evident LNM remains controversial. Yang et al, report on their study to determine the factors predictive of central compartment LNM, as well as their attempt to create a nomogram to estimate the probability of central compartment LNM in PTMC. The study population consi...

Neck manipulation may be associated with stroke
Manipulating the neck has been associated with cervical dissection, a type of arterial tear that can lead to stroke. Although a direct cause-and-effect link has not been established between neck manipulation and the risk of stroke, healthcare providers should inform patients of the association before they undergo neck manipulation. Central neck dissection in thyroid cancer
The impact of metastasized cervical lymph nodes (CLN) identified on central neck dissection (CND) in treating papillary thyroid cancer (PTC) and the extent of CND needed to reduce recurrence has not been established. Alzahrani and Xing assessed CLN metastasis and BRAF mutation in PTC by analysing data from 379 consecutive PTC patients who underwent thyroidectomy with or without CND. Increasingly aggressive tumour characteristics were found as the extent of CND was advanced, resulting in disease recurrence or persistence. BRAF mutations were more common in more extensive CND.
