Cytotoxic T Lymphocyte Antigen 4 Haploinsufficiency Presenting As Refractory Celiac-Like Disease: Case Report
We present two patients with celiac/celiac-like disease diagnosed in early childhood and refractory to the gluten-free diet. One patient had features of multi-system autoimmunity, whereas the other had celiac-like disease as an isolated clinical finding. Both patients underwent genetic testing given disease refractoriness and were ultimately diagnosed with cytotoxic T lymphocyte antigen 4 (CTLA4) haploinsufficiency. Sarcomatoid carcinoma of the stomach: A very rare and extremely aggressive tumor; a case report
CONCLUSION: Sarcomatoid carcinoma is a rare tumor. These tumors can be aggressive with a large tumoral voulume and high rate of locoregional lymph node involvement. Evidence supporting safe diagnosis of coeliac disease in children with antitissue transglutaminase titre >=5 times upper limit of normal
Conclusion 99.5% of children with TGA-IgA ≥5xULN had histological confirmation of CD, suggesting that CD diagnosis can be made securely in children with TGA-IgA ≥5xULN. If other studies confirm this finding, there is a case to be made to modify the ESPGHAN guidelines to a lower threshold of TGA-IgA for serological diagnosis of CD.
