Aravind model - Possible in the USA?
Hello, I saw some interesting articles on the Aravind Eye hospital in India and their "McDonaldization" of cataract surgery. The costs of ocular surgery have been brought to light a few months back with the Mr. Beast video. What's holding people back from adopting a similar model here in the US? Is it the liability? I am curious as to hear what your thoughts are. (Source: Student Doctor Network)

Comprehensive Practice
Is this becoming a rarity and pushed less during training? When I was undergrad 10 years ago and worked as a tech at a practice two Docs were what I would consider Comprehensive Ophthalmology. One day was for LASIK and laser procedures. They obviously had a busy cataract practice doing toric and MF IOLs. Both did a decent about of plastics like Blephs and even some Strab work on kids and adults. They had a very busy Retina clinic doing injections for your "straight forward" wet AMD and... Is John Pinto's private practice lecture series still accurate?
Came across this series from 2017, and was honestly surprised at some of the numbers he listed for mid career salaries, found in the "contract negotiation" video. John Pinto's Ophthalmology Business Management Series J. Pinto & Associates, Inc. John Pinto is the country's most-published author on the subject of ophthalmic practice management. Mr. Pinto's Business Management Seminar is posted on EyeRounds with the very kind permission of John Pinto who reserves all rights to these works. webeye.ophth.uiowa.edu ...

Feeling defeated - cataract surgery complications
Hello, Seeking advice or thoughts on improving in cataract surgery and lowering complications. I started my attending job recently and on my 9th case today had a dropped nucleus in a patient with prior vitrectomy from macular hole surgery. I think the issue started when the lens wasn’t cracking and maybe I was not careful enough when doing other maneuvers to break it into pieces and remove the nucleus. I know these things happen but I’m honestly devastated. It’s also hard because I can’t... Read more (Source: Student Doctor Network)

Ophthalmologist requiring medical clearance for cataract patients, aargh...
Patient is 93 y.o. undergoing planned cataract extraction. Ophthalmologist ordered medical clearance. We have known for over 40 years that cataract surgery is so low risk that medical conditions that don't involve an inability to lie still don't affect anesthetic risk. How much money is wasted yearly by Medicare for this nonsense? I believe that there should be a push to have this expense come from the surgeon's global fee. That would end the practice in a heartbeat. End of rant. (Source: Student Doctor Network)

Medical Retina Fellowships 2022
Which are the top medical retina fellowship programs? What are key considerations? I know believe some require medical retina fellows to take retina call along with the surgical fellows, while others do not. Any insight into which allow you to continue to operate during fellowship? I've heard some actually allow you to do cataract surgery, which is huge plus for me as I think about maintaining my surgical skills. I would like to have a "comprehensive-plus" practice in the... Cataract surgery is hard
In my second year and I’ve felt cataract surgery to be pretty challenging. I’ve done probably 16 primary cases (but 2 of them my attending or a visiting attending had to take over due to terribly loose zonules ). I feel somewhat steady with my right hand (no tremor) but maybe a small tremor and more unsteady when using my left hand. I feel I don’t have a tremor when I’m practicing by myself lol. So far I’ve basically just been using one hand and my attending will help with an extra para... Read more (Source: Student Doctor Network)

Blood glucose level to cancel cataract surgery
Thoughts on this? What would be your BG range and cut-offs? Recently I cancelled one whose BG was 400-500 mg/dL. Pt claimed to be compliant with all her diabetic medications. Talked to her PCP and found out she was lying and that she actually ran out of her humalog a month ago. No recent A1c.. given the hx I can assume her BG in the 400-500's range for the past month. Not the healthiest patient either, with CHF exacerbation in the last 3 months, known CAD, morbidly obese. Known history of... Read more (Source: Student Doctor Network)

Cataract Surgery Anesthesia Models
We are considering an anesthesia group who's model is out-of-network for all insurances for an eye surgery center. I guess there are a few insurance companies that pay beaucoup bucks for out of network charges. I personally find this a little distasteful and am worried patients will be hit with a surprise charge and this will blow back on us. That being said, I've been told by some of our anesthetists they've been proposed medicaid rates by some insurance carriers and that's not right... Read more (Source: Student Doctor Network)
