Love seeing the discussions you geniuses have about cases. So here is one I did a few weeks back. 55y 152kg M scheduled for Maxillectomy, Parotidectomy, neck dissection, and radial arm free flap. PMH: sub orbital SCC, IDDM (A1c 9.9), NAFLD s/p OLT about 7 years ago, moderate cervical stenosis on MRI, OSA (not on CPAP), HTN on beta blocker, HLD. No documented CAD, COPD, or CVA history. Burning Feet
I wanted to pick the collective mind of IPM on SDN. I have an 85 year old female. Chief complaint burning pain of bilateral feet (specifically the soles). Pain is worse at night. Worked up relatively well by her PCP, podiatrist, and two neurologists. Has had adverse effects with gabapentin, Lyrica, and duloxetine. BMP, CBC, B12, folate, TSH, A1C, all unremarkable. I need help deciding my major...
I'm currently a freshman double majoring in Biochemistry (BA) and Music Performance (BM) and honestly I don't have any time at all to do anything, ever. I need more time because I commute (1.5hr commute one way) and I'm away from home roughly 14 hours a day, 3 times a week, and around 8-10 the other 3 days of the week (I'm at school 6 days a week). Most Labor Intensive Specialties (Physically)
Here's a question I've wondered about for quite some time but haven't had the best luck getting great info on it. What are the most and least physically demanding specialties? And by that I actually mean physical labor. I know ortho requires some heavy lifting that can really strain your body and I've talked to a few GI docs who say they've suffered wrist, back, and thumb injuries.
