Georgetown vs VTC
Deciding between these two schools. Interested in either thoracic or orthopedic surgery, I know interest change but mostly leaning towards a surgical sub speciality. Surgeons vetoing thoracic epidurals
Not private practice. Not USA. Major teaching/referral centre. 40+ operating theatres. No paeds, no obs. A small part of the workload is "big" gen surg - livers/oesophagectomies/etc. The Upper GI and some hepatobiliary surgeons have decided to veto thoracic epidurals. Reasons are typically not evidence based, nor congruent across the surgeons within each department. That said... one complaint - the post-op management in our (closed) ICU - is a real issue. Intervention/surgery for widespread pain
Hi everyone, I was hoping to get some input from the group on patients with generalized pain. I'm getting a lot of referrals for patients with fibromyalgia. Challenging NSCLC-case!
Here is a challenging NSCLC case. 75 year old patient, good PS. Small primary in the left lung, mediastinum looks mostly clear on PET, only a few very small nodes. MRI of the brain shows solitary <1cm lesion. Histology come backs as adenocarcinoma in the lung, EGFR-mutated. I just SRSed the brain lesion (EGFR-mutation was not known prior to the planning CT ). Anterior Scalene Block Test for Thoracic Outlet Question
I have a thoracic surgeon sending me these for diagnostic purposes prior to first rib resection. Pts are very legit and have extensive imaging/workup with positive findings ahead of time. My question is this. On review of the literature, it seems there is some ambiguity. At times both the middle and anterior scalene muscles are blocked, and at times only the anterior scalene muscle.
