Early versus Late Caffeine Therapy Administration in Preterm Neonates: An Updated Systematic Review and Meta-Analysis
CONCLUSION: As compared with late caffeine administration, early caffeine is associated with a reduction in BPD, IVH, ROP, late-onset sepsis, and PDA in preterm neonates, albeit increased mortality. Additional RCTs are warranted to confirm these findings and evaluate whether the effect on mortality may be related to survival bias in observational studies favoring the late treatment group.PMID:37989113 | DOI:10.1159/000534497 (Source: Neonatology)

Multidisciplinary bronchopulmonary dysplasia care
The objective of this article is to review and synthesize the available literature regarding multidisciplinary care in infants and children with established BPD, and to provide a framework that can guide clinical practice and future research.AREAS COVERED: A literature search was conducted using Ovid MEDLINE, CINAHL, and Embase and several components of multidisciplinary management of BPD were identified and reviewed, including chronic care, team development, team members, discharge planning, and outpatient care.EXPERT OPINION: Establishing a core multidisciplinary group familiar with the chronicity of established BPD is r...

Risk factors for poorer respiratory outcomes in adolescents and young adults born preterm
Conclusions Even after accounting for the neonatal course, a respiratory admission during childhood remained significantly associated with reduced peak lung function in the preterm-born cohort, with the largest difference seen in those with BPD. A respiratory admission during childhood should, therefore, be considered a risk factor for long-term respiratory morbidity in those born preterm, especially for individuals with BPD. Respiratory management and outcomes in high-risk preterm infants with development of a population outcome dashboard
Conclusion More preterm infants are surviving with worse respiratory outcomes, particularly severe BPD requiring postdischarge respiratory support. Ultimately, these survivors will develop chronic respiratory diseases requiring greater healthcare resources. (Source: Thorax)

Increasing respiratory burden of prematurity: can we turn the tide?
Despite many improvements in neonatal care, prematurity remains a significant cause of childhood mortality and morbidity, and the second largest cause of death in children worldwide.1 2 Chronic lung disease of prematurity, also known as bronchopulmonary dysplasia (BPD) is the most common respiratory complication, associated with poor long-term health and neurodisability.3 Early lung protective ventilation strategies can affect lung function and respiratory outcomes long into school age.4 In this issue of Thorax, Kwok et al describe changes in the outcomes of preterm babies in England and Wales over the last decade (2010&nd...
