Persistent tachycardia in potential first-break psychosis
Hi all, I have this patient on our unit who is in their early 20s. She presents as having severe borderline personality disorder but has a strong family history of bipolar disorder I as well. How would you help this type of patient?
I have a patient who is in his early 60s who has chronic severe PTSD and I think likely either borderline traits or borderline personality disorder. This patient doesn't have any suicidal ideation. How do you determine whether borderline personality disorder patients require hospitalization?
I am noticing that, in my program, whether or not borderline patients are hospitalized after impulsive suicidal or parasuicidal acts is dealt with very inconsistently. It seems like most providers try to discern whether or not the patient is "in crisis" or "decompensated". Diagnosing borderline personality disorder
Any tips for diagnosing borderline personality disorder (or potentially other personality disorders as well...e.g. narcissistic) in patients who are not forthcoming? I think I've got a fair grasp on this. I sometimes use the McLean screener for borderline personality symptoms. But do struggle at times with patients who have a previous diagnosis of BPD, know what it is, and don't want to be diagnosed with it. [help] Borderline Personality Disorder
Anyone on this sub have a lot of experience with how to help these patients? I am not used to patients with BPD being frequently readmitted over and over again. Where I work now, there is a lot of CYA (due to past lawsuits from BPD patients?) with these patients. They get admitted, meds get changed, then discharged. Where I trained, this type of patient would rarely if ever get admitted. As far as boards, we learn admitting these patients (especially frequently) is likely more harm than... Please advise.. Applying to residency with several red flags.
I am a 4th year DO student who is interested in psychiatry with several red flags--LOA for 6 month and low board scores (no failures in boards/courses/clerkship). FYI, I took a LOA for 6 months due to personal issues (family illness/ financial difficulties/relationship problem with a partner who has borderline personality disorder). I had been dealing with these issues for more than three years before I finally decided to take a LOA before breaking down. My grades started going down... Please advise.. Borderline Personality Disorder as a "dumping ground" diagnosis for chronic suicidality?
Has anyone else noticed that clinicians seem to be assuming that chronic suicidality equals a definitive diagnosis of BPD more and more, without doing a good differential? I mean, with chronic suicidality, BPD should definitely be on the differential list, but it's not the only disorder that can lead to this (MDD, Persistent Depressive Disorder, PTSD, etc), nor is it sufficient to diagnosis BPD in and of itself. 1) Dealing with patients with Borderline Personality: Where I did residency they didn't teach much about this other than just they needed DBT then taught pretty much close to nothing about DBT nor was anyone in the department skilled in utilizing DBT as a treatment. Add to the problem that a significant portion of patients coming into the ER had a cluster B personality and the staff members including the psychiatrist didn't know how to deal with them either and usually admitted them despite... Just letting you know it's a bit of a venting post. So, I had an intake yesterday, the guy had classic symptoms of borderline personality disorder, drinks a liter of liquor daily. He demanded to be diagnosed with bipolar disorder and be started on meds for that. I got him admitted for inpatient detox. History gathering showed no major mood symptoms in absence of drinking. How do you guys handle people asking for narcotics
How do you guys handle people coming in asking for narcotics or benzos? I'm still a resident in training and I've been trying to figure out how to avoid unpleasant experience with those manipulative drug seekers with borderline personality. I've once said No straight up to their face, which made the patient storm out of the room swearing at me. Another time, a patient came in for acute visit to I&D an abscess and wanted to discuss his/her chronic pain at the end of visit. This time I tried...
