What is most revenue-generating?
Revenue generation in medicine is pretty opaque and not straightforward, but I'm curious if anyone knows what makes the most money for the hospital/medical system on average. Specifically, I'm interested in medical subspecialties only, and finding out what is the most revenue-generating and/or profit-maximizing in the inpatient and outpatient settings. I'm guessing these would be procedures like cardiac stents, endoscopies, or bone marrow transplants? Is there a way this information can be... Read more (Source: Student Doctor Network)

First week of heme onc fellowship, feeling overwhelmed
Anyone else get this feeling? I just completed first week of fellowship in the bone marrow transplant unit. Feel completely overwhelmed and stupid. Feel like I don’t know anything and all the acronyms and stuff the attending mentions all sound like a foreign language to me. Is this normal or am I behind? (Source: Student Doctor Network)

Bone pain?
I have a late 30's female patient with a history of leukemia (bone marrow transplant, chemo, nephrectomy), who has lateral hip and thigh pain. Unilateral pain. No obvious exacerbating or alleviating factors. Moderate to severe intensity, constant. She's got femoral diaphysis infarctions, several of them, and they match up exactly where she hurts. The lateral hip distribution isn't the main focus, it is the proximal and lateral thigh. She's got buttock pain as well, which was positive on... Bone pain? (Source: Student Doctor Network)

Ribavarin, BMT and nicardipine
Anyone have any experience using this drug in bone marrow transplant patients? I have an adult in my unit now the peds onc guys refuse to pass on to the adult service who's got RSV. She had been getting aerosolized ribavirin, but then we had to intubate her. Onc is pushing us to continue the treatments, but the stuff gums up his ETT and she becomes impossible to ventilate. If you do use it, how do you protect your vents from the particles? A totally random aside question: do you consider nicardipine a negative inotrope in adult patients?
