Thoraco-abdominal normothermic regional perfusion for thoracic transplantation in the United States: current state and future directions
Curr Opin Organ Transplant. 2024 Mar 15. doi: 10.1097/MOT.0000000000001143. Online ahead of print.ABSTRACTPURPOSE OF REVIEW: To provide an update regarding the state of thoracoabdominal normothermic regional perfusion (taNRP) when used for thoracic organ recovery.RECENT FINDINGS: taNRP is growing in its utilization for thoracic organ recovery from donation after circulatory death donors, partly because of its cost effectiveness. taNRP has been shown to yield cardiac allograft recipient outcomes similar to those of brain-dead donors. Regarding the use of taNRP to recover donor lungs, United Network for Organ Sharing (UNOS) ...

Expanding MELD exceptions for colorectal liver metastasis: data and policy
Curr Opin Organ Transplant. 2024 Mar 15. doi: 10.1097/MOT.0000000000001142. Online ahead of print.ABSTRACTPURPOSE OF REVIEW: While liver transplant for unresectable colorectal cancer liver metastases (CRLM) has been demonstrated to be a promising treatment in selected patients, the typically low MELD score of these patients and lack of exception points may lead to challenges in receiving a deceased donor liver for transplant.RECENT FINDINGS: Several studies have shown improved outcomes in select patients with CRLM who undergo liver transplant, and several trials are ongoing and will conclude in the next several years. MELD...

Accelerated versus Standard Rehabilitation after Meniscus Allograft Transplantation in the Knee
J Knee Surg DOI: 10.1055/a-2274-6914Meniscus allograft transplantation (MAT) is a proven treatment option for patients with symptomatic irreparable meniscus deficiency. When patients are adherent to prescribed postoperative restriction and rehabilitation protocols, outcomes after MAT are considered good to excellent. However, nonadherence to standard protocols is common and can be associated with undesirable outcomes and patient dissatisfaction. Based on demonstrated safety for early weight-bearing following MAT in conjunction with significant advances in graft preservation and surgical techniques, our joint preservation c...

Clinical outcomes of intestinal transplant recipients receiving maintenance basiliximab, sublingual tacrolimus and prednisone: A case series
CONCLUSION: The combination of monthly basiliximab, sublingual tacrolimus, and prednisone is an effective novel maintenance immunosuppression in intestinal transplantation. A larger and more extended study duration would be necessary to thoroughly assess the safety and sustained benefits of the novel maintenance immunosuppression regimen.PMID:38480040 | DOI:10.1016/j.humimm.2024.110787 (Source: Human Immunology)

Endomyocardial Biopsy in Clinical Practice: The Diagnostic Yield and Insights from a 5-Year Single-Center Experience
CONCLUSIONS: With advances in cardiac imaging modalities, EMB is not routinely indicated for the diagnosis of cardiomyopathy. However, EMB is still an important tool for diagnosing specific cardiac diseases and could be crucial for confirming the diagnosis. EMB is generally safe if performed at experienced centers.PMID:38479703 | DOI:10.1016/j.hjc.2024.03.004 (Source: Hellenic Journal of Cardiology)
