Chronic cough question from Pulmonologist to GI doctors
Hello GI doctors I have an honest question about community GI setups regarding getting esophageal manometry and 24 hour pH probe Bravo studies. As a pulmonologist I see a lot of chronic cough. After I have gone through the checklist of asthma/COPD/bronchiectasis/structural lung disease and done all of my PFTs, FENOs, bronchoprovocation testing, and CT chests, I am often still confronted with GERD I barium esophagrams and at times I get lucky with GERD but this is not always the case... Medical waiver denied AF, chances of approval with Army?
Hey MilMed! Long story short, I've been applying for AF HPSP since October. Responses have been SLOW. Today, I finally heard back that my medical waiver was denied. I had childhood asthma (last prescription at age 15, no incidence since age 9, and lots of time in sports (including Swimming) in HS, and a recent (06/2022) psoriasis diagnosis). Before I put effort into trying with the Army, I'm wondering if anyone has had success getting either condition waived as a disqualifier? Thanks in... Read more (Source: Student Doctor Network)

Is xanax contraindicated in Asthma / Copd exacerbation
Is there any data that restricts the use of low dose xanax (0.5mg) in mild asthma exacerbations (not requiring o2) Can they still take to help them sleep. Is it a complete contraindication? (Source: Student Doctor Network)

Recently developed long covid after having covid a month ago. now suffering from severe fatigue, memory issues and asthma like reactions... Devastated by this turn of events. can I get through a prelim surgery year, do well enough to get good letters while battling long covid or should I forfeit the position and focus on health?... I likely will not have this opportunity as a reapplicant... Read more (Source: Student Doctor Network)

“Office” or “For emergency” med prescriptions
Hey everybody, Let me start by saying that I am 100% NOT talking about any controlled meds, and I am posting this from my regular easily-identifiable SDN handle that I’ve had for 10+ years, so there is no question about anything shady. I’m an ER doc living and practicing in FL. Because of my job and training, it’s not unlikely that if I were presented with an emergency while off work (asthma/anaphylaxis/OD/etc) I’d be able and qualified to do a little good in the world by treating... Vent Management primer
Definitely have a knowledge gap here, but figured brushing up here would be useful since we end up with boarding ICU vented patients. Usually I'll set the tidal volume to 400-500cc, RR 15-18, higher PEEP for ARDS/CHFer types, minimal for asthma patients, and let the ICU handle the rest. Has worked well for me so far. Hoping @sylvanthus and some of the CCM folks can jump in here. What kind of specific vent settings do you guys prefer to use for common situations, such as CHF, ARDS... Interesting case
Had an interesting thing happen in the cardiac OR yesterday: 50 year old man, severe MR w/ P2 prolapse, scheduled for MV repair. Otherwise pretty healthy and well compensated, had albuterol on his med list but didn't carry a formal diagnosis of asthma and denied using his inhaler in years. At the end of the bypass run, cross clamp comes off, start to pace, did a little recruitment then started trying to ventilate but was getting very high airway pressures. In the surgical field, the lungs... Read more (Source: Student Doctor Network)
