Biologic Augmentation of Rotator Cuff Repair: PRP May Be of Significant Benefit, Whereas Atelocollagen Can Not Be Recommended
The field of orthobiologics continues to advance at a rapid pace, and theoretically holds some promise to augment the biologic healing response in rotator cuff repair. However, the clinical evidence for use of substances such as platelet-rich plasma for rotator cuff repair remains inconclusive. Atelocollagen, as a synthetic collagen substitute, has been proposed as another alternative to provide more collagen substrate for healing, but outcomes data with this technique is lacking. As biologic augmentation options continue to push the envelope on indications, due diligence is required to carefully examine options for safety...

Arthroscopically assisted closed reduction for displaced lateral humeral condyle fractures over 4 mm in children.
We read with interest the publication by Weng and colleagues1. The same authors2 reported in 2022 that open reduction and percutaneous pinning (ORPP) remained their “gold standard” for displaced and rotated lateral humeral condyle fractures (LHCF), whilst the authors2 encouraged readers to be prepared for the time-consuming learning process of closed reduction and percutaneous pinning (CRPP) for these fractures, without mentioning that Weng et al1 themselve s had already abandoned ORPP and CRPP by September 2020 in favour of the arthroscopically assisted procedure1 (AAPP). Arthroscopic inferior leaf meniscectomy of the involved anterior horn in the lateral meniscus horizontal tear via an accessary extreme far anteromedial portal
An accessory extreme far anteromedial portal can improve visualisation and ease inferior leaf meniscectomy in patients with lateral meniscal anterior horn horizontal tears. However, the therapeutic outcomes of... (Source: BMC Musculoskeletal Disorders)

A guide to facilitate the creation of  a femoral tunnel for arthroscopic ligamentum teres reconstruction: a three-dimensional computed tomography study
J Hip Preserv Surg. 2023 Aug 19;11(1):13-19. doi: 10.1093/jhps/hnad027. eCollection 2024 Jan.ABSTRACTThe ideal femoral tunnel passing through the centre of the femoral neck targeted to the footprint of the ligamentum teres (LT) is established during the LT reconstruction surgery with the free-hand technique. We aimed to quantitatively determine the entry site and define the angular orientation of the ideal femoral tunnel with its relevance to the femoral valgus angle (FVA) and the femoral anteversion angle (FAA) to facilitate the creation of an ideal femoral tunnel during the LT reconstruction surgery. A total of 60 random...

Does arthroscopic or open washout in native knee septic arthritis result in superior post-operative function? A systematic review and meta-analysis of randomised controlled trials and observational studies
ConclusionsAW was associated with equivalent to superior post-operative function and lower requirement for further intervention compared with OW. Results need to be interpreted cautiously, taking into consideration the methodological and clinical heterogeneity of the included studies.Systematic review registrationPROSPERO 2022, CRD42022364062. Psychological Resilience Promotes Positive Outcomes After Surgery
The significance of psychological factors in orthopaedic surgery has long been underestimated. High psychological resilience plays a significant role in achieving a positive postoperative outcome in terms of mental health, pain, and functional outcomes. This underscores the need for a more holistic approach to patient care, one that considers not only the physical aspects of treatment but also the emotional and psychological well-being of patients. This may involve implementing strategies to enhance resilience, providing support resources for coping with the challenges of surgery and recovery, and fostering open communicat...

Advancing Pediatric Care: The virtualKIDS Experience in Nursing-Led Audio-Visual Clinical Services
Creat Nurs. 2024 Apr 11:10784535241245667. doi: 10.1177/10784535241245667. Online ahead of print.ABSTRACTDuring the COVID-19 pandemic, the escalating trend of pediatric patients, particularly non-urgent cases, going to the emergency departments (EDs) in New South Wales, Australia, prompted the establishment of virtualKIDS, a nursing-led telehealth service. This service, initiated in June 2021, operates 24/7 and provides comprehensive care through audio-visual consultations emphasizing a patient-centered approach. Three elements-COVID-19 Outpatient Response Team (CORT), virtualKIDS Acute Review (vKAR), and Virtual Urgent Ca...

Discharge Opioid Dose Indirectly Associated With Functional Outcomes 2 Weeks After Shoulder and Knee Arthroscopy in a US Military Sample
CONCLUSION: Excess opioid prescribing was common, did not result in improved pain alleviation, and was associated with poorer physical function and sleep 14 days after surgery. As such, higher prescribed opioid doses could reduce subacute functioning after surgery, without benefit in reducing pain. Future patient-centered studies to tailor opioid postsurgical prescribing are needed.PMID:38602453 | DOI:10.1093/milmed/usad495 (Source: Military Medicine)

Prices and complications in hospital-based and freestanding surgery centers
CONCLUSIONS: The higher prices charged in HOPDs for the 3 ambulatory procedures were not balanced by better quality-as measured by rates of procedural complications-compared with procedures performed in nonhospital ASCs.PMID:38603532 | DOI:10.37765/ajmc.2024.89529 (Source: The American Journal of Managed Care)
