Case discussion
70ym has stable c spine fracture (don’t remember the details but neurosurgery wants in Miami J at all times and to revisit outpatient in a few months kind of thing). Presents with cardiogenic shock from critical Aortic stenosis. On low dose NE, BP 90/60s but not intubated. Let’s say cardiologists wants to do a TAVR, your choice as to general vs MAC. Would you do general upfront (AFOI) or do MAC? If MAC, how worried would you be to emergently intubate someone with a high risk procedure and... Read more (Source: Student Doctor Network)

My on-line editorial (http://www.bcs.com/pages/news_full.asp?NewsID=19792127) discusses the criteria that need to be fulfilled to meet the diagnosis according to the latest ESC guidelines. I also discuss paradoxical low-flow low gradient severe AS, and the potential for confusing this new entity with moderate stenosis.I look forward to your commentsN...

ECG in LVH: Quiz 3
This 69 years old woman with aortic stenosis was hospitalized because of frequent premature beats and dizziness. While in the emergency room, premature beats were partly suppressed by the use of intravenous antiarrhythmics. She was then started on oral antiarrhythmics. On examination she had an irregular heartbeat, the typical ejection systolic murmur of aortic stenosis and S4 gallop. Her ECG showed the following. What is your interpretation? Please click to enlarge the view. (Source: Doc2Doc BMJ Cardiology)
