anal cancer poor chemo candidate
I have an elderly patient with a T3N0 anal squam. Not a great 5FU/MMC candidate per med onc. If you were to try to twist the arm of med onc into something radiosensitizing, what would you consider? xeloda or weekly cis? I see this case series using just 5FU. Any thoughts are appreciated. Changes you would like to make to your practice if you are 'chosen' for APM
I'm interested in seeing if anyone's practices are discussing, as a group, changes to their treatment guidelines if/when APM comes. As a below-the-diaphragm GI-radonc; I'm at 5 fractions for most things besides anal cancer and post-operative treatments; but I am really interested in the following: Managerial - any private groups/employed groups/academic groups having meetings, lectures, etc on this? Including the ischiorectal fossa in anal cancer - yay/nay?
Australians say always include all of it. American and British say do not include unless clear evidence of spread through levator and even then only some of it. What's the answer? Does anyone actually follow the Australian advice and include the whole thing for every anal case? Doing something inbetween (giving more margin on muscle)? (Source: Student Doctor Network)
