Challenging NSCLC-case!
Here is a challenging NSCLC case. 75 year old patient, good PS. Small primary in the left lung, mediastinum looks mostly clear on PET, only a few very small nodes. MRI of the brain shows solitary <1cm lesion. Histology come backs as adenocarcinoma in the lung, EGFR-mutated. I just SRSed the brain lesion (EGFR-mutation was not known prior to the planning CT ). The thoracic surgeons performed a mediastinoscopy this week to rule out any mediastinal involvement, they resected two nodes... Read more (Source: Student Doctor Network)

Logic behind different concurrent chemotherapy regimens
Hello everyone, As I'm studying for boards (on GI cancers now), I have been noticing the plethora of concurrent chemotherapy regimens that have been tried over the years and I'm trying to make sense of this information. As general rules of thumb, do we expect certain concurrent chemotherapy regimens to work better in adenocarcinoma compared to SCC or vice versa? (5FU/cisplatin/carboplatin/etoposide/xeloda/epirubicin/leucovorin) For example, why would you treat both SCC and... Read more (Source: Student Doctor Network)

Complex Anal Case
Love to know what others think. Got a lady with extramammary pagets of the perineal area with an associated invasive adenocarcinoma involving the entire anal canal and peri-anal soft tissues with extension to the left labia and multiple 1/5-3 cm bilateral inguinal nodes with no distant disease. These tend to be bad actors. Planning chemo, preop chemorads and APR with plastics for recon. I am curious to know what people would do with the inguinals. This is not your typical SCC of the anal... SBRT dose for bone osteocondensing vs osteolytic lesions?
Dear colleagues, I have a patient with metastatic prostate adenocarcinoma, that responded very well to systemic treatment except for one painful osteocondensing lesion of the left ilio-ischial bone. North Carolina Medical Board: Adenocarcinoma of Appendix
The Board is concerned that on December 31, 2017, a surgical pathology specimen was submitted to you, following an appendectomy with colostomy performed on Patient A. Within two weeks, you communicated a verbal preliminary report finding a likely adenocarcinoma of the appendix. Your final report was not submitted until more than six months later on July 14, 2018. Stuttering During Rotations
I have a question from attending physicians and residents. I have a pretty bad stutter. I am an IMG. My stutter is bad to the point that I have blocks and my head starts moving when I speak and I don't make eye contact. When I am on the phone people usually get frustrated or think the signal is bad. In medical school, I never did any presentations except twice which went very badly. All the classes we had to present I talked to teachers to just give me a C. I have heard in America residents... Foreign Medical Graduate Seeking help!
Hello, everyone! I have been a long time reader of this forum. I have recently come to America. I am a permanent green card holder. I graduated from medical school in 2017. I studied in an EU medical school. However, due to life circumstances, I had to come to America (long story).
