Laryngeal Dysfunction Manifesting as Chronic Refractory Cough and Dyspnea Laryngeal Physiology in Respiratory Health and Disease
Chest. 2024 Mar 18:S0012-3692(24)00396-9. doi: 10.1016/j.chest.2024.03.026. Online ahead of print.ABSTRACTTOPIC IMPORTANCE: Laryngeal dysfunction as a cause of chronic refractory cough (CRC) and episodic dyspnea is often missed which results in unnecessary testing and delays in diagnosis. Understanding laryngeal roles in breathing and airway protection can help to appreciate the propensity to laryngeal dysfunction with aging, chronic lung disease and sleep apnea.REVIEW FINDINGS: The human larynx is a complex muscular structure that undertakes multiple roles of breathing, vocalization, coughing and swallowing. To undertake ...

Causal relationship between irritability and asthma: a bidirectional two-sample Mendelian randomization study
CONCLUSION: The causal relationship between irritability and asthma was analyzed through MR analysis. Irritability increased the risk of asthma. GERD might play an important mediating role in this relationship.PMID:38506573 | DOI:10.1080/15257770.2024.2330594 (Source: Nucleosides, Nucleotides and Nucleic Acids)

Response
We appreciate the interest of Fujiyoshi and Teshima1 in our randomized trial2 comparing conventional myotomy (CM) and oblique fiber-sparing myotomy (OFS) for achalasia cardia. The incidence of significant reflux esophagitis ( ≥LA B) was not statistically different between the 2 groups. Fujiyoshi and Teshima1 postulate that oblique fibers were possibly spared in both groups, resulting in a similar incidence of post–peroral endoscopic myotomy (POEM) reflux esophagitis in our study.2 Additionally, they hypothesize that in real-world clinical practice, oblique fibers are dissected at the collar part of the gastroesophageal...
