abilify to counteract prolactin increase with risperdal
Do you guys do this often? Ive done this in the past a while ago, and have used abilify in conjunction with clozapine. Risperdal has been amazing for one of my bipolar patients (shes completely stable on it now) but shes having persistent increased prolactin and galactorrhea. Was considering adding 5mg abilify to try and offset prolactin increase but may have her see endo too. (Source: Student Doctor Network)

When do you switch antipsychotics in the inpatient setting?
Curious how long you guys trial a patient on an antipsychotic before switching course and changing meds? We are under the gun to move people of course, so I often find myself 7-8 days in and scared to change meds because we'll be starting from square one all over again. I also know that we may not get peak efficacy for 6-20 weeks out. I find myself often going to olanzapine or risperdal, then changing to the other. Occasionally abilify if there's documented positive response in the... Read more (Source: Student Doctor Network)

Weird case. Treatment Resistant Depression but after a snake bite
Got a patient that was already seen by a great clinician and she couldn't get this patient better. Was tried on all the conventional meds. A few SSRIs, SNRIs, Lamotrigine, Lithium, atypical antipsychotics (and yes the better ones for depression such as Aripiprazole, Cariprazine) , Bupropion, augmentation agents such as Buspirone, Auvelity, Esketamine, TMS (not ECT...yet). After several meetings she finally said something that hit me like a ton of bricks. She told me she never had mental... Read more (Source: Student Doctor Network)

Is this unethical?
Is it immoral to ask ur inpatient nurses for low doses of commonly prescribed second generations and first generation antipsychotics like haldol or abilify 5 in order to understand what exactly I'm giving my patients . There's something icky about the idea of being given meds that aren't prescribed but my intention isn't abuse either obviously , I just feel llke I want to know what it is I'm giving , like truly . Anorgasmia Treatments?
Looking for some input from the hive mind here. 33yo with schizophrenia. Had him on inpt for about 4 months before he was finally stabilised on Abilify Maintena 400mg and olanzapine 10mg. But he’s had Anorgasmia since being on the meds. Not sure which one did it as he came to me on both. No erectile difficulties, he just can’t finish. I tried bupropion and mirtazapine, no effect. Right now I’ve left him on trazodone 50mg qhs and amantadine 100mg BID (amantadine to counteract D2 blockade... Read more (Source: Student Doctor Network)

I'm sad
I just needed to vent. Sorry in advance. Looking for sympathy. Admitted 40s M with schizoaffective do. His med list from the midlevel provider outpatient: Trinza, Seroquel, Abilify, Rexulti - all at healthy Christian doses. If that was not enough, also takes Adderall, Elavil, and a squirt of Requip. I thought he was pulling my leg when he reported that. Sure enough med rec came back spot on. to his report. I feel sorry that he is going to have a few days of not knowing if he is... Options for clozapine-resistant schizophrenia?
I have a patient on the long term ward who transferred from a local short term ward, presented with persisting persecutory delusions and command hallucinations with accompanying violence risk, dx was schizophrenia. He came in on Risperidone 4mg BID, had previously been on Haloperidol, Olanzapine, Aripiprazole, and Lurasidone, Didn't want to up the Risperdal because anecdotally I haven't seen much success with doses above 8mg. Given he had failed multiple antipsychotics and the fact he was... Read more (Source: Student Doctor Network)

Tachycardia with stimulant? When to discontinue?
Hi, CAP fellow here. I have a 7 yo kid who I inherited from another doctor with a history of PTSD, ADHD and Intermittent Explosive Disorder. He's been on many different antipsychotics in the past due to behavioral outbursts. I have him on Focalin XR 20 mg qAM and Focalin XR 5 mg qNoon (insurance does not cover IR for reasons beyond me), Clonidine 0.1 mg qHS and Abilify 2 mg qAM (he was on a higher dose before but trying to lower as he is quite overweight). At any rate, the parent has been... Tachycardia with stimulant? When to discontinue? (Source: Student Doctor Network)

Ethical Dilemma
I came across an issue today. I inherited a patient with a Dx of MDD with psychotic Fx. Upon speaking to her it was very clear she was malingering to get disability. This became more clear with chart review. She was not on an antipsychotic. My attending agreed with me that she is malingering but states because she already carries this diagnosis but is not on an antipsychotic and I should start 2.5mg Abilify otherwise the previous notes could cause potential trouble. I disagree as I do not... Ethical Dilemma (Source: Student Doctor Network)

Lamictal dosing for bipolar disorder
I was reviewing some information about a new patient I will be seeing in a couple of weeks, and was surprised to see that the patient reports taking 500 mg of Lamictal for bipolar disorder. Other current medications include Abilify 15 mg, Elavil 25 mg, Celexa 40 mg, and Adderall 10 mg. I have several patients on Lamictal, and usually stop around 100-200 mg. I have taken it up to 250 mg on occasion, but I don't think I've ever prescribed higher than that and certainly not 500 mg. I... Lamictal dosing for bipolar disorder (Source: Student Doctor Network)

What are things you do or do not call for in retail pharmacy
I've seen some rphs call for Ventolin and proair switches. Would you call md if hydrocortisone 2% cream is written? That doesn't even exist so yeah Would you call when trying to fill abilify for a patient taking risperdal or just ask the patient on that? (Source: Student Doctor Network)

How do you guys sort through which EMR drug interactions are legit?
I always click through zofran for SSRI patients (the most common one), but I get worried how cavalier I've become. The other day I gave diflucan to a woman on aripiprazole and some other AP....she didn't die, but I feel like we get alarm fatigue with the bull**** interaction checks. Is there a list of "real deal" med interactions? (Source: Student Doctor Network)
