Funding to disseminate and implement patient-centered outcomes research (PCOR) findings into innovative, evidence-based interventions to reduce healthcare disparities in chronic health conditions. Priority populations include inner city; rural; low income; minority; women; children; elderly; and those with special healthcare needs, including those who have disabilities, need chronic care, and need end-of-life healthcare.
