What is health care provider fraud? It is intentionally fraudulent or wasteful practices conducted by health care providers, including physicians, rehab centers or labs. Examples include add-on lab tests, pass-through billing, predatory marketing practices or billing for services not rendered. It can be found in all health care claims data.
