Accessory Spleen and Endometriosis: Images in Gynecology
The patient was a 29yo G3P2012 woman with abnormal uterine bleeding and chronic pelvic pain who presented for total laparoscopic hysterectomy. She had a history of hereditary spherocytosis managed with open splenectomy at age 13. Preoperative imaging showed a small, mobile uterus with no focal fibroid. A “circumscribed, isoechoic nodule with apparent internal vascularity measuring 1.1 × 0.8 × 0.9 cm which appears separate from the ovary” was noted on pelvic ultrasound (Fig 1).
