It is 25 years since the publication of Milan criteria for liver transplantation in hepatocellular carcinoma (HCC), but the optimum management of patients progressing beyond Milan criteria on the waiting list remains a controversial topic. Ferrer-F àbrega and coworkers aimed to determine whether the policy of allowing limited progression beyond enlistment criteria permits acceptable outcomes. Patients were retained on the waiting list despite exceeding enlistment criteria if they did not present with macrovascular invasion, extrahepatic sprea d or cancer-related symptoms.
