We thank Shujaath et  al1 for reading and commenting on our article in the At the Focal Point section regarding large blood vessels within pancreatic pseudocysts.2 Although there are several approaches to the management of pancreatic pseudocysts,3-5 for some patients with severe conditions, especially walled-off necros is, endoscopic drainage may be complicated by postprocedural bleeding, recurrence, or infection. We previously developed a new endoscopic approach to drain pancreatic pseudocysts called endoscopic transgastric fenestration (ETGF).
