While scaphoid excision combined with Four Corner Arthrodesis (FCA) or ProximalRow Carpectomy (PRC) are commonly-used salvage procedures to treat type two and three Scapholunate Advanced Collapse (SLAC) and Scaphoid Nonunion (SNAC)-induced degenerative arthritis, controversy persists over which treatment provides superior outcomes. We searched for articles comparing range of motion, grip strength, complications requiring reoperation, conversion to wrist arthrodesis, pain, and disability of shoulder and arm scores between FCA and PRC-treated patients.

ConclusionStudies show clear beneficial effects of exercise in PsA on disease activity, on well-being, and on comorbidities, and they seem to outweigh the risk of enthesitis induced by mechanical stress. Further investigations are necessary to confirm these results and to precise the modalities of exercise.Key Points• Psoriatic arthritis patients have a sedentary lifestyle• Physical activity has beneficial effects on disease activity, well-being and reduced some cardiovascular risk factor in psoriatic arthritis• Risk of enthesitis and flares is low with exercise in psoriatic arthritis

ConclusionWe have identified the post-zygotic p.Ser171Phe NLRC4 variant as a plausible cause of the disease in the enrolled patient. Functional and structural studies clearly support for the first time itsgain-of-function behavior consistent with previously reported NLRC4 pathogenic variants. These novel evidences should be considered in the diagnostic workup of patients with uncharacterized AID starting during adulthood.
