51-Year-Old Man With Ascites and Abdominal Pain

A 51-year-old man presented to the emergency department with 2 days of abdominal pain. He was in his usual state of health when he experienced progressive epigastric abdominal pain radiating to the back, which was worsened while lying supine. Nausea, emesis, and anorexia accompanied the pain. Symptoms did not improve with acetaminophen or omeprazole. He denied any fever, hematemesis, melena, or hematochezia.
