37-Year-Old Woman With Shortness of Breath

A 37-year-old woman presented to the emergency department with the chief complaint of shortness of breath of 5-hours duration. The onset was abrupt without additional associated symptoms. Her medical history includes ruptured intracranial aneurysm treated with surgical clipping 1 year previously with no neurological sequelae. Three weeks ago, she underwent open reduction internal fixation of a left ankle fracture. She has no other comorbidities, and her only medication is an oral combined estrogen-progestin contraceptive.
