We concur with the expert panel of the American Radium Society that there are significant areas of controversy, particularly in relation to radiation therapy dose and fractionation in bladder cancer.1 A recent meta-analysis of 2 pivotal bladder radiation therapy trials advocates 55 Gy in 20 fractions (55 Gy/20 fx) as the standard of care for bladder preservation,2 but we urge caution for the following reasons.
