Total Arterial Revascularization: It Is Time for a Dedicated Team

We have read with great interest the article by Velez and colleagues1 focusing on the relevant role of a programmatic focus started at Johns Hopkins Hospital, aiming to increase the use of multiple arterial grafts (MAG) in patients requiring isolated coronary artery bypass grafting (CABG). Despite the encouraging evidence of MAG in coronary patients, and the recommendations reported in the latest guidelines on CABG, the use of MAG, especially the use of radial artery (RA) in addition to bilateral internal thoracic arteries (BITA), is still an underused approach.
