Comment on: Persistent urothelial differentiation changes in the reconstructed exstrophyic bladder: congenital or acquired dysfunction of the epithelial barrier?

We would like to commend the authors on their work to further describe the urothelium in the exstrophic bladder. Analyzing 115 CBE specimens, they found persistent urothelial differentiation changes in the majority of bladders at the time of subsequent continence procedure after a successful primary closure. At present, the ability to foresee which bladders will grow capacity to gain continence continues to elude the pediatric urologist, and our best predictor continues to be a successful primary closures.
