A Young Patient with EBV-Positive Nasopharyngeal Carcinoma: Dissonance of Dose, Variance of Volume, and Tussle of Timing

The patient is a previously healthy 18-year-old man with no medical history and previously normal development. He presented with rapidly progressive bilateral level II neck masses. He was treated with antibiotics without response. He then developed bilateral nasal obstruction and snoring. At that time, his general practitioner referred him to an otolaryngologist. Visible on flexible nasophayngoscopy was a bulky mass on the right posterior nasopharyngeal wall with ulcerated tissue occluding the right side of the choana and submucosal swelling extending inferiorly beyond the oropharynx into the supraglottic region.
