High-Dose Chemoradiation Irrespective of Age

This 18-year-old with Epstein –Barr virus (EBV)+ nasopharyngeal carcinoma can be treated using either the pediatric or the adult paradigm. Given his locoregionally advanced presentation, induction chemotherapy (IC) is strongly considered. The Children’s Oncology Group ARAR0331 enrolled patients up to age 19. Patients receive d 3 cycles of IC (cisplatin and fluorouracil), followed by concurrent high-dose cisplatin and radiation therapy. Complete/partial responders received 61.2 Gy, and those with stable disease received 71.2 Gy, yielding an estimated 5-year cumulative incidence of local and distant relapse of 3.7% and 8. 7%, respectively.
