Bilious  peritoneal dialysate in a peritoneal dialysis patient

A 71-year-old man on peritoneal dialysis for end-stage kidney disease secondary to hypertensive nephropathy presented with vague right upper quadrant abdominal pain and fever. The initial peritoneal dialysate was mildly cloudy but with a slight brownish hue; he was treated for peritoneal dialysis –related peritonitis with intraperitoneal cefazolin and ceftazidime. However, the peritoneal effluent become dark a few hours after admission (Figure 1). The surgical team was consulted immediately for suspected perforated viscus.
