Mitral annular disjunction: A myocardial abnormality too?

Mitral annular disjunction (MAD) is a structural abnormality in which there is atrial displacement of the mitral valve hinge point away from the left ventricular myocardium. This can be seen as a separation between the atrial wall-mitral valve junction and the crest of the ventricular myocardium. First identified in the 1980s [1,2] MAD was originally described in association with myxomatous mitral valve disease [2] and has since been linked to increased risk of arrhythmias and sudden cardiac death [3].
