Redefining cancer of unknown primary: Is precision medicine really shifting the paradigm?

An enigmatic entity, cancer of unknown primary (CUP), has evolved alongside the development of modern oncology. The first mention of cancer with “unknown primary” dates back to 1946 [1]. Since that time, a shift in the definition and diagnosis occurred, associated with the emergence of ever more sophisticated diagnostic tools which aim to trace biological and molecular similarities to the elusive, possibly dormant or regressed, primary [ 2].
