Painless Visual Loss in a Septuagenarian

A septuagenarian with a history of Crohn disease, coronary artery disease, hypercholesterolemia, and transient ischemic attack presents with sudden, painless visual loss. Examination of a temporal artery biopsy specimen shows chronic, nongranulomatous inflammation. Reexamination of the biopsy specimen 2 weeks later shows abundant transmural deposits of amorphous eosinophilic acellular material that exhibit apple-green birefringence consistent with amyloid deposits; IgM and lambda light chain restriction are also observed.
