Postwash total motile sperm count and intrauterine insemination pregnancy rate: how low can you go?

Quoting monthly success rates of natural or medicated intrauterine insemination (IUI) cycles to couples with infertility can be a lesson in bracing oneself for disappointing people. Depending on which study you reference, the success rates usually range from 10% to 20% per month. Even in the group with the best prognosis —a truly anovulatory patient with no history of infertility partnered with a fertile male—the fecundity rate is approximately 15% per ovulation event (1). Patient reactions are often a mixture of disillusionment and honest surprise at the limitations of their own reproductive ability.
