An enlarging ischiorectal mass

A 74-year-old man presented with a 6-month history of a slowly enlarging mass in his right gluteal area. His history includes a partial right nephrectomy for renal clear cell carcinoma 13 years before this visit. Colonoscopy performed 6 months earlier did not identify any abnormalities. He appeared well-nourished, and physical examination revealed a 20 cm diameter soft, freely movable, right-sided ischiorectal mass without any obvious communication with the rectum or anus. Magnetic resonance imaging showed a vertically oriented dumbbell-shaped, predominantly T2 hyperintense and T1 hypointense mass measuring 16 cm in length and 6 cm in diameter.
