Repeat hepatectomy for recurrence of colorectal liver metastases: To whom and what to give?

In this issue of the American Journal of Surgery, Wong and colleagues form OHSU report a series of patients from a single institution who underwent a repeat liver resection following recurrence of previously resected colorectal liver metastases (CRLM). Of 338 patients who underwent curative intent surgery (CIS) via resection with or without thermal ablation, 147 recurred and 38 patients (11%) were able to receive a second CIS. This cohort was the focus of the study. Thirty-six of patients received preoperative chemotherapy prior to repeat CIS while 29% had postoperative chemotherapy.
