Allogeneic hematopoietic cell transplantation (HCT) is a curative treatment option for patients with serious, non-malignant blood disorders including aplastic anemia and hemoglobinopathies. Rejection of the allogeneic graft has been a significant complication especially in heavily transfused patients, with rejection probabilities in the range of 35-60% in the earliest transplant series. Rejections were seen despite the use of human leukocyte antigen (HLA)-identical sibling donors and were attributed to sensitization to minor non-HLA antigens via transfusions1.
