Over time outpatient palliative care has changed, extending to management of patients with chronic disease. Well-studied outpatient clinic models (particularly in heart failure and oncology) improve access and outcomes, especially if patients are referred early. However, there is a lack of data assessing the impact of embedding palliative care within general internal medicine.
