Salvage of cephalomedullary nail cutout with the variable angle proximal femoral plate.

Cephalomedullary nail (CMN) is a highly diffusion technique in the treatment of the trochanteric fractures in the elderly. Excellent results have been published, but not free of complications. One of them is the perforation of the lag screw through the superior cortex of the femoral head or neck, followed by rotation and varus collapse of the head and neck fragment (cut-out). This complication occurs between 3% and 7% of cases [1, 2], showing a pseudoarthrosis of the fracture and a very important bone loss in the upper neck and femoral head.
