Non-response to acute vasodilator challenge and prognosis in heart failure with pulmonary hypertension

In left heart disease (LHD), pulmonary hypertension (PH) develops in response to a passive backward transmission as a result of increased left ventricular filling pressures.1 The long-standing consequences of passive pressure transmission end into a vascular disease process driven by a combination of factors additive to the high pressure load such as vasoconstriction, chronic venous congestion and activation of smooth muscle growth factors, altogether promoting vascular remodeling, increased resistive load, right heart failure and worse outcome.
