Effectiveness of a Patient-Centre Medical Home model on diabetes and other clinically relevant outcomes among primary care patients diagnosed with type-2 diabetes in Sydney, Australia

The prevalence of type 2 diabetes (T2D) continues to rise worldwide, and its strong association with increased morbidity and mortality is well documented [1,2]. In Australia, it is reported that the prevalence rates have tripled over the last 25 years despite pharmaceutical and technological advancements in medicine [3]. The National Health Survey (NHS) 2018 reported that diabetes accounted for 11% of all deaths in Australia, and that there are currently 1.3 million self-reported diabetes cases and an additional estimated 0.5 million undiagnosed cases [4].
