Caspr2 autoantibody-associated Morvan syndrome predating thymoma relapse by 30 months

A 63-year-old Caucasian male was diagnosed with thymoma B2 in 2002 during work-up for ocular myasthenia gravis. In 2015, thirteen years after thymectomy, he experienced difficulties playing the trumpet (he was an avid trumpet player). Mild myasthenia due to pleural recurrence of thymoma was discovered (stage IV-A). He underwent induction chemotherapy (3 cycles of carboplatin/paclitaxel), followed by a left-sided extrapleural pneumonectomy. Once discharged and recuperated, he went on vacation to the Dominican Republic, during which he developed loss of appetite and a vague abdominal pain.
