Terry's nail due to uncontrolled diabetes mellitus

A 35-year-old Japanese woman presented with nausea and frequent vomiting that had begun the previous day. She was slightly confused. Laboratory blood testing revealed a plasma glucose level of 319 mg/dl and serum hemoglobin A1c level of 11.4%. Venous blood gas analysis revealed a pH of 7.33, bicarbonate level of 19.6 mmol/L, sodium level of 144 mEq/L, chloride level of 103 mEq/L, and anion gap of 21.4. The patient was thus diagnosed with diabetic ketoacidosis, admitted to the hospital, and treated with insulin and intravenous fluids.
