Spinal Cord Reirradiation:Balancing Benefit Against Risks

A 58-year-old patient without comorbidities received a diagnosis of breast cancer 20 years previously (T2N0M0, ER positive, PR positive, HR negative). Five years later a bone metastasis of the first thoracic vertebral body was diagnosed. Prior irradiation included the right chest wall, including regional lymph nodes (45 Gy in 1.8 Gy fractions; field border = vertebral column, including currently involved right-sided nerve roots) 20 years ago and the seventh cervical to third thoracic vertebrae 5 years later (39.6 Gy in 1.8 Gy fractions).
